1. Eligibility You must be at least 18 years of age or the legal age of majority in your jurisdiction to use our services. By booking a session, you affirm that you meet this requirement.

2. Health Acknowledgment and Informed Consent You acknowledge that EMS training involves physical exertion and electrical stimulation, which may pose health risks. By participating, you confirm:

• You have consulted a physician or are medically cleared to engage in EMS training.

• You do not have any contraindicated medical conditions, including but not limited to pacemakers, epilepsy, heart conditions, or pregnancy.

• You understand the risks and voluntarily assume all responsibility.

3. Liability Waiver To the fullest extent permitted by law, EMS SLC and its employees, contractors, and affiliates are not liable for any injury, loss, or damage that may result from your use of our services. You agree to release and hold harmless EMS SLC from all claims arising from participation.

4. In-Home Service Policy You agree to provide a safe, clean, and accessible space for your session. EMS SLC reserves the right to cancel or terminate a session without refund if the environment is deemed unsafe or inappropriate (e.g., aggressive pets, smoking indoors, etc.).

5. Payment Terms Payment is due prior to or at the time of service. We accept most major payments. Package purchases are non-refundable unless otherwise stated. Pricing is subject to change with notice.

6. Cancellation and Rescheduling Clients must cancel or reschedule sessions at least 24 hours in advance. Cancellations made with less than 24 hours' notice may be charged in full. No-shows will be billed as completed sessions.

7. Use of Images and Testimonials With your consent, EMS SLC may capture photos or videos during sessions for promotional purposes. You may opt out by notifying us in writing.

8. Service Changes and Availability EMS SLC reserves the right to modify, discontinue, or suspend services at any time without prior notice. Efforts will be made to notify affected clients in advance.

9. Code of Conduct Both clients and staff are expected to act respectfully and professionally. Any form of harassment or misconduct may result in termination of services.

10. Service Area Our services are currently available within [insert radius or area, e.g., Salt Lake City and surrounding areas]. Travel fees may apply for locations beyond this zone.

11. Dispute Resolution Any disputes arising under these Terms will first be attempted to resolve through mediation or arbitration. Jurisdiction will be governed by the laws of the State of Utah.
